Description:
During a recent EMS call on 4/29/25, Derek failed to complete the full patient assessment before initiating transport. Specifically, vital signs were not obtained prior to movement, and the patient's history was incomplete on arrival at the hospital. While no harm resulted, this could have impacted continuity of care. Derek and I reviewed patient care protocols the following day.
